your first appointment...

For
your convenience, we have provided you with the necessary forms
so you can complete them and bring them with you to your first
appointment.

Please
also bring with you your doctor's prescription for physical
therapy as well as all of your insurance cards. This includes
auto insurance cards and claim and adjuster information, or
worker's compensation claim and adjuster information if applicable.

Please
wear comfortable clothing and footwear, as well as appropriate
clothing for the therapist to access the area(s) to be treated.

Please
bring any diagnostic imaging or testing you have had (i.e. x-ray,
MRI, CT scan, bone scan, etc.) as well as the report of the
results. If you have a post-surgical protocol or report, please
bring this with you. This important information will help the
therapist more accurately understand your condition and safely
and appropriately provide treatment.

Please
bring your medication list with you. It is important that the
physical therapist know what types of medications you are taking
in order to properly and safely provide treatment.

If
your insurance requires a referral from your primary care physician's
office, please be sure to call and obtain this once you have
scheduled your initial evaluation with us. Please note that
a referral is not the same as a prescription. A doctor's prescription
is necessary to provide physical therapy services.

Patients
are responsible for knowing their own insurance policy and benefits.
However, for your convenience, your insurance benefits will
be verified and provided to you on the date of your first appointment.
If you would like to know your benefits, simply call the number
on the back of your insurance card for member services and ask
for outpatient physical therapy benefits in an office setting.
Please be sure that we are in-network with your insurance, and
verify if any co-pays, co-insurances or deductibles apply. All
co-pays, co-insurances and deductibles are due on the date that
services are provided, or once your insurance company has processed
the claims and determined your portion of responsibility. Please
